Because we are gathering your history and completing a comprehensive assessment, your first visit may take a little longer than a routine appointment.
What to Bring to Your First Dental Appointment
If available, please bring or provide:
- Your medical and dental history
- Information about previous dental treatment
- A list of current medications
- Your dental insurance information
- Any questions or concerns you would like to discuss
- Completed Health History Form (download below)
- Previous dental records and X-rays may also be helpful and can often be transferred directly from your former dental office.
Following your examination, we will discuss our findings with you and create a personalized care plan, including recommendations for future examinations and dental hygiene visits.
